Johor Is Becoming Part of the Singapore Lifestyle Equation — Healthcare May Be the Next Big Driver
Historically, johor medical interests have linked Johor Bahru to weekend shopping, dining, property investment and short getaways for many Singaporeans.
But that perception may be changing.
However, a CNA report highlighted a significant rise in Singapore patients traveling to Johor Bahru for johor medical treatment.
Moreover, this includes patients using MediSave for eligible procedures at Regency Specialist Hospital.
Additionally, the hospital said the number of Singapore patients using MediSave has doubled since the COVID-19 pandemic.
This raises a broader question:
Could access to healthcare become another important reason for Singaporeans to consider Johor not simply as a destination, but as part of their longer-term lifestyle, retirement and property planning?
Healthcare Across the Causeway Is Already Happening
Additionally, Regency Specialist Hospital in Johor Bahru receives about 150 to 200 international patients each day.
Among its MediSave users, about 90% are women.
Moreover, obstetrics, gynecology and maternity-related services are among the common treatments sought.
Singapore citizens and Permanent Residents have been permitted to use MediSave for certain elective hospitalizations and day surgeries at approved overseas healthcare facilities since 2010, subject to eligibility conditions and withdrawal limits.
The attraction is understandable.
Additionally, Johor is geographically close.
It has an established private healthcare sector and can offer a substantially different cost structure compared with Singapore.
Research published by the ISEAS – Yusof Ishak Institute estimated potential savings for selected medical procedures performed in Malaysia versus Singapore ranging from approximately 24% to 74%, depending on the treatment.
However, price should never be the only consideration.
Patients still need to assess medical quality, specialist experience, insurance coverage, hospital accreditation, follow-up treatment, travel requirements and the total cost of care rather than simply comparing procedure prices. CNA also noted that many Singapore insurance plans may not cover overseas private treatment in the same way as treatment received locally.
The Bigger Picture: Johor’s Healthcare Ecosystem Is Expanding
The more interesting story may therefore be what is happening around healthcare itself.
Johor’s private healthcare sector is already supported by several major hospital groups, including facilities operated by KPJ, Columbia Asia, Gleneagles and Regency.
ISEAS has identified healthcare as one of the sectors that could benefit from the development of the Johor-Singapore Special Economic Zone (JS-SEZ), alongside increased cross-border economic activity and population growth.
Significant new healthcare developments are also planned.
For example, Sunway Healthcare intends to develop a 300-bed medical center in Iskandar Puteri, while Thomson Medical has announced a major integrated healthcare development near the JB city center incorporating a hospital, specialist facilities, aged care, hospitality and life-sciences components.
This suggests that healthcare may become increasingly integrated with residential, retirement and lifestyle developments across Johor.
Then Comes the RTS
Connectivity could be one of the biggest catalysts.
The Johor Bahru–Singapore RTS Link is expected to significantly reduce the friction involved in traveling between Woodlands North and Bukit Chagar.
CNA reported that the RTS is slated to begin operations in January 2027 and noted that easier cross-border travel could further support demand for healthcare services in Johor.
The broader implication goes beyond medical tourism.
When crossing the border becomes more predictable, Johor becomes easier to consider for:
- retirement living
- second homes
- long-term residential living
- healthcare and wellness
- business expansion
- family visits
- lifestyle spending
That changes the way property buyers may eventually evaluate location.
Instead of asking only:
“How close is this property to Singapore?”
buyers may increasingly ask:
“How close am I to healthcare, transport, groceries, restaurants, community facilities and the lifestyle I want?”

Healthcare becomes particularly important when considering retirement.
A beautiful condominium or landed property may look attractive today, but as we grow older, accessibility to doctors, hospitals, pharmacies, rehabilitation services and caregivers becomes increasingly important.
Johor’s aging population — combined with Singapore’s aging demographic — means demand for healthcare and aged-care services is expected to grow substantially over the coming decades.
For Singapore families exploring Johor as a potential retirement destination, this means property selection should not be based purely on price appreciation or rental yield.
The surrounding ecosystem matters.
A suitable retirement property might ideally provide access to:
Healthcare + Community + Transport + Daily Amenities + Security + Family Connectivity.
These factors may ultimately matter more than whether a development has the largest swimming pool or the most impressive clubhouse.
Does This Mean Everyone Should Seek Treatment in Johor?
No.
Healthcare decisions are personal and should always be made after considering medical suitability, insurance protection, treatment complexity, continuity of care and professional medical advice.
Likewise, not every Johor property automatically becomes a good investment simply because healthcare demand is increasing.
But the larger trend is worth watching.
Johor’s relationship with Singapore appears to be gradually evolving from one based mainly on shopping, tourism and property investment into something much broader.
